    "That the research proves out..." You are actively doing damage to people's physical health. When respectable universities start offering medical degrees in your dumbass massage therapy with salt on it, call it a discipline. You are actively destroying progress towards an understanding of medicine and science. Real people waste billions of dollars on your bullshit that was debunked decades ago, and continues to be. I'm sorry, I have a sister who is breaking her bank to pay for someone to do what you advertise as medical care. You are a bad person.

    Unfortunately there are classes of chiropractors, because the system of education and training have historically lacked. The problem is some chiros simply suck at diagnosis. However others are excellent clinicians. Some within the profession are working to change bring up the standards. Even a condition such as disc herniation cannot be generalized. It must exist on a case-by-case basis (WHERE is the herniation? Is it central? Lateral recess? Neuralcanal? How large is it? Is it acute, chronic, or acute aggravation superimposing chronic? What is the patient's history? What are the patient's flags/risk factors? What are the patient's comorbidities? Are there any superimposing or underlying conditions? Has the patient been screened for any of these potential complicating factors? These types of questions must be answered prior to treatment.) Even as a Chiropractor, I avoid a chiro who always pushes treament > diagnosis. I have discovered everything from fracture to bone cancer to spinal cord compression because I DIAGNOSE FIRST, treat second. The moron who treated Rogan was just that. No responsible doc would treat a patient beyond 2-4 weeks without escalating care d/t nonresponse, which means he did not adequately diagnose. I always fight the damn insurance companies on their 'no MRIs' bullcrap, if indeed MRI is indicated. A good doc has to advocate for their patients to arrive at an accurate diagnosis, prognosis, treatment plan.

    I have been taking Atorvastatin, aka: AtorvaSatan for almost a year. I was first prescribed 40mg daily. Within 2 months are had terrible unfamiliar back pain. I flew to Thailand and back and probably stood for 75% of the flight. After a lipid test in early October showed my cholesterol to be critically low, I started backing off. By December I was taking 10mg 3-4x a week, and my back pain was nearly gone. By the new year it was forgotten. Fast forward to April this year. I fired my cardiologist who had refused to discuss lowering the statin and never followed up with me on the lipid test. My new cardiologist came as a reference from my new functional medicine doctor. I told him what I had done and why. He was afraid my cholesterol would be too high (his concern, not mine). He asked me to take 20mg Daily for the next few weeks and then take another lipid test. I agreed, but in a little over a week the back pain came back. I took the lipid test and immediately went back to my statin routine. That was a few days ago, and my back is still killing me. By the way, my lipid results were great according to the cardiologist. But I want my ldl higher.

    Physical Therapists/ therapy techs can do anything that a chiro can do that is of actual scientifically proven benefit, and they can do it as a medical doctor / under the supervision of one, which means they have all of the evidence based care that anyone should need. No one's saying medical care is perfect, but the system in place to ensure that it is in fact evidence-based is the best option we have and certainly isn't surpassed by any non-medical chiropractic association. Also, name one research study that says chiro is the best first option for care? Even one, not that one actually proves your point, because you would need a meta analysis of a wide range of studies to actually make that claim.

    Myofascial pain syndrome is the MOST COMMON manifestation of chronic pain. Myofascial pain syndrome is characterized by active myofascial trigger points, Myofascial trigger are taut bands in skeletal muscle or muscle knots. Continuing to use muscles with trigger points cause both peripheral and Central sensitization ie allodynia and hyperalgesia. Dry needling is the most effective way to treat active myofascial trigger points. There is always an underlying cause why the formation of myofascial trigger points continue. But let's really get into the science. Moseley, himself wrote the excellent chapter on pain in the equally excellent 4th edition of the text Clinical Sport Medicine(2012). Here are some Moseley excerpts: “Trigger points are present in all patients with chronic pain and are thought to reflect sensitization of nociceptive processing in the central nervous system… Patients with active trigger points present with persistent regional pain… These patterns clearly reflect central convergence of peripheral inputs, which further implicates the central nervous system in their generation and contribution to a pain state.” Moseley GL. Pain: why and how does it hurt? Brukner & Kahn’s Clinical Sports Medicine. North Ryde, NSW: McGraw-Hill Australia Pty Ltd; 2012 "elimination of myofascial trigger points is an important component of the management of chronic musculoskeletal pain” and “we have found ischemic compression and dry needling to be the most effective" According to Dommerholt, all treatments fall into one of these two categories or both: a pain-control phase and a deep conditioning phase. During the pain-control phase, trigger points are deactivated, improving circulation, decreasing pathological nociceptive activity, and eliminating the abnormal biomechanical force patterns. During the deep conditioning phase, the intra- and inter- tissue mobility of the functional unit is improved, which may include specific muscle stretches, neurodynamic mobilizations, joint mobilizations, orthotics, and strengthening muscle pubmed.ncbi.nlm.nih.gov/7850884/ Myofascial pain J R Fricton. Baillieres Clin Rheumatol. 1994 Nov. MFP is a regional muscle pain disorder characterized by localized muscle tenderness and pain and is the most common cause of persistent regional pain. The affected muscles may also display an increased fatiguability, stiffness, subjective weakness, pain on movement and slightly restricted range of motion that is unrelated to joint restriction. MFP is frequently overlooked as a diagnosis because it is often accompanied by signs and symptoms in addition to pain, coincidental pathological conditions, and behavioural and psychosocial problems The difficulty in managing MFP lies in the critical need to match the level of complexity of the management programme with the complexity of the patient's situation. Failure to address the entire problem, through a team approach if needed, may lead to failure to resolve the pain and perpetuation of a chronic pain syndrome. pubmed.ncbi.nlm.nih.gov/16259310/ A review of myofascial pain and fibromyalgia--factors that promote their persistence Robert D Gerwin. Acupunct Med. 2005 Sep Chronic muscle pain (myalgia) is a common problem throughout the world. Seemingly simple, it is actually a difficult problem for the clinician interested in determining the aetiology of the pain, as well as in managing the pain. The two common muscle pain conditions are fibromyalgia and myofascial pain syndrome. The problem these syndromes pose lies not in making the diagnosis of muscle pain. Rather, it is the need to identify the underlying cause(s) of persistent or chronic muscle pain in order to develop a specific treatment plan. Chronic in myalgia may not improve until the underlying precipitating or perpetuating factor(s) are themselves managed. Precipitating or perpetuating causes of chronic myalgia include structural or mechanical causes like scoliosis, localised joint hypomobility, or generalised or local joint laxity; and metabolic factors like depleted tissue iron stores, hypothyroidism or Vitamin D deficiency. Sometimes, correction of an underlying cause of myalgia is all that is needed to resolve the condition. www.sciencedirect.com/topics/nursing-and-health-professions/myofascial-painMyofascial Pain Syndrome Andrea L. Nicol MD, MSc, ... F. Michael Ferrante MD, in Essentials of Pain Medicine (Fourth Edition), 2018 Myofascia l pain or regional musculoskeletal pain is one of the most common pain syndromes encountered in clinical practice. Iv had right decompression surgery for stenosis. On my discharge sheet it just says right decompression surgery but in the pamflet they give me it only mentiones laminectomy and nothing about laminotomy. So basically I don't know which I have had. My question is first is it possible to have a laminectomy only on one side and if you do dose that mean that the spinous process has been removed in the way that it would if the procedure was done on both sides. This is so confusing. Hope you can help.

    • @Theevidencebasedchiropractor
      @Theevidencebasedchiropractor Місяць тому

      Hello, while I can't speak to your case specifically, if your notes mention a laminectomy, then it would seem as though that was the procedure performed. Regarding the spinous (removed or not), that would typically depend on the surgeon and how they chose to best address your specific challenge. I would recommend asking the surgeon; they should be able to provide better specifics for your surgery.
